To prepare for your first visit to Dr. Anderson's office, please...
Read the Privacy Practices form first. Then, print out the Acknowledgement form (indicating that you have read the Privacy Practics) and sign. Next, print out and complete the ABOUT YOU form pages. Be sure to bring both the Acknowledgement and About You forms ( a total of 4 pages) with you to your first appointment.
1. Please read this form... you may wish to print it out for your records, but you do not need to bring it with you to the office.
2. Print the form below and sign it to acknowledge that you have read the Notice of Privacy Practices document (in colum at left). Bring the signed form to the office on your first visit. ACKNOWLEDGEMENT FORM
3. Please complete the About You (The Patient) form and bring all three pages to the office on your first visit.
